puppy paceing in the pen

i have a 4 mounth old puppy that is pacing back and frouth in the pen 75 % of the time have a pup in the pen with him makes no dif pen is 12 x 10 he paces like a tiger at the zoo .
i trun him out every afternoon for 3 to 4 hours he is on aoutomatic feeder but will not put on wight i have seen this one time before on a 2 year old female but never one this young i do not know what to do help me if you can .

Had one to do the same thing one time and it ended up being an inner ear infection that was causing her balance to be off. She paced so badly in the pen she wore her feet raw.
